The words get translated. Nothing else does.
A language review does not find this, because the words are correct. It appears when someone uses the course. The layout, the artwork, the navigation and the interactions still behave as if the course were English.
The person responsible usually cannot check it. The project manager does not read Arabic, and a translator checks the words, not the build.
That is where localization becomes course engineering.
